Responsibilities:
To coordinate provision of home health care services to assigned patients that reflect the Agency mission and standards.
To ensure quality and safe delivery of home health services.
To ensure that nursing care services are provided in accordance with Federal, State and local standards, guidelines and regulations.
To demonstrate support for achieving the mission of the Home Health Agency.
Qualifications:
An active LPN license in the state in which practicing.
